THE BIG PICTURE

Your benefits, in perspective.

Pilot design score · Published comparisons below · Grades are not market percentiles.

100% weighted scoring coverage. Missing information remains unknown.

One plan, individual in-network protection, employee-only and family deductions, and core benefit availability, with optional retirement and PTO shown separately.

Overall grade weights: plan value 30%, financial protection 30%, employee premium contributions 30%, breadth 10%.

Premium scores describe employee contribution competitiveness. They do not measure household affordability, which also depends on income and spending for care. Employee-only and family contributions each make up half of the premium category.

01High

Review missing core benefits

Why: This benefit is confirmed as not offered. Missing: Short-term disability.

Next step: Price the missing benefits before your next renewal.

02Opportunity

Review employee-only payroll costs

Why: Employee-only premium contributions are $1,800 per year, $434 above the published employee-share average of $1,366.

Next step: Price a larger employer contribution at renewal.

03Opportunity

Keep family payroll costs in view

Why: Family medical premiums cost employees $650 per month ($7,800 per year). The published median is $7,700 and 75th-percentile contribution is $14,000 for US private sector · 25–99 employees · all plan types (2025).

Next step: Review employee needs alongside the family premium, deductible and out-of-pocket maximum.

The averages in the table are means. Family contribution scoring uses the published US private-sector distribution shown here, across medical plan types. Family means in the table are context and do not anchor that score. HSA-specific means remain useful context. Other components use their own stated references. A mean is not a median or an exact market rank.

Benefit Grader policy: $0 earns 100; published P25 = 90/A, median = 80/B, P75 = 70/C and P90 = 60/D. Scores interpolate in dollars between these cutoffs; above P90 the score is 60 × P90 / annual contribution. Letters and interpolation are our policy, not AHRQ grades or exact percentile ranks. Rounded survey cutoffs have sampling uncertainty.

Published standard errors describe sampling uncertainty in each estimated cutoff, not the spread of individual plan costs. Survey cutoffs are rounded. No exact percentile rank or confidence interval for this plan is inferred.

Comparison definitions and limitations

Industry and employer sector are not matched. National or all-plan-type data may be used where a closer reliable comparison is unavailable. AHRQ and KFF size bands refer to the whole firm. Margins of error are approximate 95% intervals, calculated as 1.96 × the published standard error. They describe uncertainty in an average, not the range of individual plans.

To compare your own premium split, you need the total plan premium as well as the employee deduction. This Snapshot does not collect total premiums, so your employer contribution and percentage are not inferred.

BLS assumes workers in an occupation can choose single or family coverage; these figures are not weighted by actual coverage elections. They should not be combined with the AHRQ or KFF dollar averages.

These national figures are not matched to your state, industry or firm size. BLS size groups describe an establishment, usually one workplace, rather than the entire company.

No national BLS dollar average is shown in this version. Dollar figures conditional on requiring an employee contribution are not substituted for averages that include zero contributions.

National medical-cost context · Milliman, HCCI and Aon

These findings describe different populations, years and cost measures. They are not combined into an average and do not affect your grade. Average out-of-pocket spending is not a plan's out-of-pocket maximum.

Milliman · 2026 projected health-care cost

$8,460 per covered person in an average employer-sponsored PPO, including $1,266 in projected out-of-pocket spending.

Modeled total health-care costs include employer payments, employee contributions and out-of-pocket spending. These are not premium-only amounts. Figures are net of pharmacy rebates.

HCCI · 2023 observed claims spending

$6,743 per covered person under 65 in medical and prescription claims spending, including $923 paid out of pocket.

Based on employer-insurance claims from 2023, published in October 2026. Premiums are excluded; pharmacy spending is before manufacturer rebates.

Aon · 2026 reported premium cost

$17,562 per employee in combined employer and employee medical and prescription premiums, including $3,130 from employee payroll contributions.

04Opportunity

Consider company support for core benefits

Why: Vision is employee-paid.

Next step: Price a company contribution to make coverage easier to afford.

05Opportunity

Review the worst-case cost

Why: The individual in-network out-of-pocket maximum is $7,000, or $6,000 after guaranteed employer HSA funding. The pilot B point from KFF's grouped distribution is $5,000. Pilot review range: $5,000–$7,999.

Next step: Check whether employees could manage this amount in a difficult year.

Benefit value uses employee coinsurance because visits go toward the deductible first. Financial protection separately accounts for deductible and out-of-pocket exposure.

Family payroll premiums use published MEPS-IC contribution percentiles with a disclosed Benefit Grader grading policy. Other components retain their pilot reference curves. Letters, interpolation and weights are our policy, not survey-endorsed grades or exact market ranks. Means and medians are labeled separately.

A letter requires at least 80% coverage in every category and 80% overall, confirmed plan structure, and valid scoring references. Letters use unrounded scores. A score near a letter boundary is shown to one decimal so rounding does not imply a higher grade. Retirement and PTO do not change core breadth.

Family contributions use published contribution percentiles: the 25th percentile maps to 90/A, the median to 80/B, the 75th to 70/C and the 90th to 60/D. Zero employee contribution maps to 100, with linear interpolation between these points. Above the 90th percentile amount, the score is 60 × that amount ÷ the entered annual contribution; there is no fixed dollar amount that forces a zero score. These score and letter assignments are Benefit Grader policy. Interpolated scores do not establish an exact market percentile. Other component references map to 80/B under their pilot curves. Category weights and letter thresholds are design choices. Missing data remains unknown. Entered facts have not been independently verified.
